Swiss health insurance and rehab: what basic cover pays

Swiss basic health insurance pays for rehab when treatment is medically necessary and takes place on the general ward of a clinic on a cantonal hospital list (Fedlex: KVG Art. 25). It does not normally pay for one-client or luxury private programmes, which are almost always self-paid.

The short answer

Supplementary insurance can add a private or semi-private room, free choice of doctor and access to more clinics. If you do not live in Switzerland, Swiss basic insurance does not apply to you.

  • Basic insurance: medically necessary care in listed clinics
  • Supplementary insurance: room type, doctor choice, wider clinic access
  • Self-payment: one-client and luxury programmes
  • International clients: home insurer or private funds

How basic insurance works

Basic health insurance, known as KVG in German and LAMal in French and Italian, is compulsory for everyone who lives in Switzerland. It covers inpatient treatment for addiction and mental health conditions when a doctor confirms it is medically necessary. It pays for inpatient care only in hospitals on a cantonal hospital list (Fedlex: KVG Art. 39). Larger clinics such as Forel Klinik and Privatklinik Meiringen accept basic insurance for eligible residents.

What you pay yourself

With basic insurance, you share part of the cost of a rehab stay.

  • Standard deductible: CHF 300 per year (FOPH, 2026)
  • Retention fee: 10% of costs above the deductible, up to CHF 700 per year (FOPH, 2026)
  • Hospital contribution: CHF 15 per day of a hospital stay (FOPH, 2026)

You can choose a higher deductible in return for a lower premium. The hospital contribution is charged on top of the deductible and the retention fee.

What supplementary cover adds

Supplementary insurance (Zusatzversicherung in German) sits on top of basic cover. Depending on the policy, it can pay for a semi-private or private room, treatment by a doctor of your choice, and care in clinics outside your canton. Many policies limit psychiatric or addiction stays, for example to a number of days per year or to clinics on a contract list. Two plans with similar names can pay very differently for rehab, so read the terms.

Cost approval before admission

Before a planned stay, most insurers want to approve the cost in advance (Kostengutsprache in German). The clinic or your doctor usually sends the request with the medical reasons, the planned length of stay and the room type. Without an approval, you may have to pay the full bill yourself. Ask the clinic to confirm in writing what the insurer has agreed to pay, and for how many days. Extensions often need a new approval.

Why luxury clinics are usually self-pay

One-client and luxury clinics are not on cantonal hospital lists, and their prices are far above insurance tariffs. A week at Paracelsus Recovery costs CHF 110,000 according to the clinic (Paracelsus Recovery, 2026). Clinic Les Alpes quotes CHF 60,000 a week as its entry price (Clinic Les Alpes, 2026). Some supplementary or international policies may reimburse part of the medical cost. In practice, clients often pay up front and claim back what they can. The guide to the cost of rehab in Switzerland compares the price bands, and the luxury vs standard rehab guide explains what the extra cost buys.

Cover for international clients

Swiss basic insurance covers only people who live in Switzerland. What you can claim depends on your home country system or your international private medical insurance; our guide for patients coming to Switzerland from abroad covers the practical steps. Some international policies cover planned treatment abroad with prior approval. Others exclude addiction treatment completely, or pay only for a limited number of days.

How to check your cover

Ask your insurer and the clinic the same questions and compare the answers. Clinic admissions teams deal with insurers every day and can often send the request for you. Do not let paperwork delay care when it is urgent. The page on alcohol rehab in Switzerland covers the clinics and programmes for the most common reason for admission.

  • Is the clinic on my canton's hospital list?
  • Which room type does my policy pay for?
  • How many days are covered per year?
  • Do I need a cost approval, and who sends it?
  • What will I pay myself in total?

Frequently asked questions

Does Swiss basic insurance pay for alcohol rehab?

Yes, when the treatment is medically necessary and the clinic is on the cantonal hospital list. Cover is for the general ward, and you pay your deductible and share of costs.

Can I use supplementary insurance at a one-client clinic?

Sometimes, for part of the medical cost. Most of the price of a one-client programme is not covered, so plan to pay privately and claim back what your policy allows.

Is there a waiting time for clinics covered by basic insurance?

Waiting times can occur at larger clinics with insurance contracts, especially for specialised programmes. Private self-pay clinics usually admit faster, often within days.
